    Writing a compounded prescription.

Everything you need to write a clean compounded prescription: strength conventions, click-based sigs, standardized supplies, and ePrescribing. Questions? A pharmacist answers the phone.

    The Essentials

    Anatomy of a compounded prescription

Five elements make a compounded Rx unambiguous. Include all five and a clean Rx moves straight through pharmacist review into the compounding queue — no clarification callback needed.

    1

    Drug + strength

    Estradiol 0.5 mg/mL topical cream

Name the active(s) and concentration. For combinations, list each component (e.g., Bi-Est 80/20 2.5 mg/mL).

    2

    Base / dosage form

    Topical cream in Topi-CLICK

Specify the form — cream, gel, capsule, troche, suppository — and the dispenser if metered dosing matters.

    3

    Sig with clicks or units

Apply 2 clicks (0.5 mL = 0.25 mg) topically once daily

Include both the click count and the mg equivalent so the patient and pharmacist stay aligned.

    4

    Quantity + days supply

    Qty: 15 mL · Days Supply: 30

Write volume (mL) or count (capsules/troches) plus a standard 30, 60, or 90-day supply.

    5

    Refills

    Refills: 2

Compounded BHRT is typically refilled after follow-up labs — most providers write 2 refills with a 3-month recheck.

    Metered Topical Dosing

The Topi-CLICK system: clicks, concentrations, supplies

Topi-CLICK applicators meter by volume — we compound the concentration to your patient's dose, so the sig stays constant and titration only changes the strength. Choose the dispenser by desired days supply.

    30-Day Supply

    Topi-CLICK 35

    2 clicks (0.5 mL) daily

    15 mL fill — 0.25 mL/click

Standard monthly fill. Concentration = prescribed daily dose ÷ 0.5 mL.

    60-Day Supply

    Topi-CLICK 35

    2 clicks (0.5 mL) daily

    30 mL fill — 0.25 mL/click

Same dispenser and concentration as the 30-day, larger fill volume.

    90-Day Supply

    Topi-CLICK 140

    2 clicks (1 mL) daily

    90 mL fill — 0.5 mL/click

Larger dispenser, half the cream concentration — same daily dose in 1 mL.

Sample Rx: "Estradiol 0.5 mg/mL topical cream in Topi-CLICK. Sig: Apply 2 clicks (0.5 mL = 0.25 mg) topically once daily. Qty: 15 mL. Days Supply: 30. Refills: 2." The same 2-click sig works at every strength — only the cream concentration changes.
